There is only one field to complete and that is to replace the “Full Name” field with your full name.  Once you click on the “submit” button, scroll back down the page and you will be given the option to download a PDF copy of your certificate. There is no need to email your copy as Josephene and Karen will automatically receive a copy of your certificate via email to provide with the final report. Thanks again for your participation!
